The evaluations' objectives are to assess participant and instructor satisfaction with specific programs and increase subsequent program effectiveness. Evaluations, whether written or electronic, must be solicited from participants and instructors. If the instructor is actively involved in developing the program materials, it is not necessary to request an evaluation from the instructor.
The evaluation must include the five required topics detailed in Standard No. 14; however, sponsors may include other questions in the evaluation. Even if there are no stated prerequisites for the program, the topic needs to be added in the evaluation form to comment on whether or not the participant believed that specific knowledge, skill, or experience was needed before taking the program.
Add Evaluation Form
Follow these quick steps to create and publish your evaluation template:
1. From the left navigation bar, click Author> Evaluation Editor> Add Evaluation Form
2. Give your template a title. Naming your evaluation form will help identify it if you're creating multiple custom evaluations.
3- Now, you can begin adding questions. Please note that there are two tabs: course and instructor specific evaluation. Click the "course" tab to enter questions about the course content; a live preview will appear on the right-hand side of the screen. You may choose to receive the learner's feedback in the form of ratings, multiple-choice, single line, or paragraph responses. These can also be optional or required by checking the "Required" checkbox. Repeat the same process for the instructor tab.

Please note: The Sponsor's details along with the instructor's name are a placeholder on your template until associated with an event. When in use, the template will automatically pull in the correct information from the specific event.
3- Once you've created all of your questions, you can click Next on the lower right navigation bar, and it will take you to the full preview. If everything looks good, click Activate on the lower right navigation bar to make the new evaluation form available for your events.
